Documents to present to enroll your child in

- Filled in application form (Anexo I)
- Copy of child's family record book” (Libro de Familia).
- Copy of passport/identity card (DNI) for the child (in case he/she has).
- Copy of parents' passports/identity card (DNI), and one of any person collecting the child.
- Medical certificate issued by a doctor, but not necessary using the official form, to prove that the child has no contagious disease.
- Copy of health insurance card.
- Copy of vaccination/inmunization record.
- A passport photograph (only if the child is two or older).
- Filled in authorization form (anexo II), which specifies that child's parents or legal guardians accept or not that the school takes pictures of the child in order to publicize its activities.
- Filled in rules acceptance form (anexo III), where parents claim to have read and understood the rules described in Anexo VIII. For that purpuse, these rules should be read before applying on our website (www.guarderiacostateguise.com) or ask us for a printed copy.
